The evidence ladder:
how to read any
supplement claim

25 OCTOBER 2026 · ADRIAN VAN IERSEL · ~4 MIN READ

The testimonial is a persuasive genre: a person, a transformation, a discount code. What it never tells you is the only thing that matters — whether the claim survives testing. Day 41 of the programme installs a four-rung ladder for exactly that job, and once it's in your head, every "energy" ad reads differently.

The four rungs

Bottom rung: mechanism — a plausible story from a lab or an animal model. A reason to ask questions, not proof it helps tired people. Second: observational — groups of people studied as they live; associations with caveats attached. Third: trials — the intervention actually tested, in a defined population, against an endpoint that was named in advance. Top rung: systematic review in people like you — many trials pooled, and the population matching yours. The recall item is simply: name the four rungs. The decision that precedes it: buy on testimonials, or climb?

Who was studied — and what was measured

The top rung carries two qualifiers, and they do heavy lifting. Early research suggests NR/NMN supplements raise blood NAD+ levels — but whether NAD+ supplements improve energy or function in healthy adults is not shown to work in practice; a higher blood level is not a better outcome. CoQ10 is established as effective in primary CoQ10 deficiency — a specific clinical population — while the same compound's effect on statin-associated muscle symptoms is not shown in practice. Early research suggests urolithin A improved some muscle-endurance measures in small trials in middle-aged and older adults. Same shop shelf, four different verdicts. "Who was studied" is not a detail.

The rung nobody advertises

Established, and worth its own paragraph: supplements can interact with medicines. That single fact is why the programme's Restore action is to list questions for your clinician or pharmacist rather than to start anything — and why the Optimize action stops at assessing one claim with the ladder. The programme never gives dosing advice; this rung belongs to professionals who can see your full medication list.

The honest summary of the shelf

Early research suggests most supplements marketed for "energy" lack trial evidence in healthy adults. That sentence — a PROMISING label doing modest, careful work — describes the aisle better than any storefront. The ladder doesn't tell you what to buy. It tells you how confident any claim is allowed to make you.
